    VA SMILES PARENT LETTER

We are excited to announce that Smile Virginia will be visiting Forest Middle School on January 22, 2025 to offer dental care to students. Dental care includes a complete dental exam, x-rays, cleaning, fluoride and sealants when necessary, and in some cases, restorative care such as fillings.

Participation is optional. If you want your child to participate, go to www.myschooldentist.com and complete the permission form by December 18, 2024. Paper copies of the permission form are available from the school nurse or in the main office.

Smile Virginia will see children with private dental insurance and Virginia FAMIS/Medicaid. Please be sure to complete the insurance information on the permission form. If your child does not have dental insurance, you may choose the self-pay or donated care option on the permission form.

Please note: Smile Virginia will visit Forest Middle School and Jefferson Forest High School on January 22nd but they require a minimum number of students to participate or they will cancel the dental clinic. You will be notified if the dental clinic is canceled.

Taking care of your child’s teeth is an important part of keeping them healthy, in school and learning! Please reach out to your school nurse if you have any questions.

This is an important message from Forest Middle School for all parents, guardians, visitors and volunteers. Our school, along with all Bedford County school facilities, has transitioned to a new check-in and check-out system called Raptor.  It is very important to know that a driver’s license scan will be required for anyone who will be checking out a student or for anyone who will be in the building as a visitor or volunteer. Thank you for your assistance and we look forward to a great year.
