There has been a change in School-Required Vaccines for the upcoming school year. Effective July 1, 2021 HB 1090 amendment changes the current immunization requirements for students enrollment.

The change for middle school is for our upcoming rising 7th grade students who will need to be immunized for meningococcal ACWY before entering 7th. Please watch for more information to come home with your child.

The requirement for Tdap and HPV remains unchanged.

Attention Parents:


SchoolCare “CareDox”: Be sure to log on to https://www.caredox.com/category-faq/parents/ to connect with our electronic medical record system. Parents will receive electronic notifications about clinic visits through SchoolCare and will not receive paper/pencil notes.


The Authorization to Administer Medication form is good for one school year. These forms are available on our school website, in the office, and clinic.

Parents that have a child with medical concerns, asthma, diabetes, food or bee sting allergies must meet with your school nurse with authorization forms complete the week before school starts in August.


General Clinic Information

**Students should not call or text parent to go home. Student should ask teacher for permission to go to the clinic to see the nurse when not feeling well.**

  • Your student must have a completed and signed emergency card on file in the clinic. This information is essential should an emergency arise. If there are any health concerns there must be medical documentation/ medications/ supplies in the clinic (these documents are also available in the clinic).

  • Medication forms and care plans are only good for one school year. If your student has a medical condition (i.e. - life threatening allergy, asthma, diabetes, etc.) or takes daily or as needed medications, a new form will be required for the 2022-2023 school year. These forms can be found by clicking the "Health Forms" link on the left of the page.

  • Additionally, please register your child/children in CareDox. This is our electronic charting system that notifies you of treatment provided to your child/children. CareDox is HIPAA & FERPA compliant. If you would like to register, email CareDox at activation@caredox.com

  • Hearing and Vision Screening will take place this fall for ALL 7th grade students, more information will be provided soon.

Student Wellness Policy


The objectives of our Student Wellness Policy are to promote student health and reduce childhood obesity. Teachers are using other options for instruction/incentives rather than food items. Attention to individual student allergies and health care plans within the class population is required. No sharing of outside food and drinks among students. Do not send cupcakes, candy, or food treats for birthday celebrations or class parties.(For a complete list of recommended snacks see "Health Forms" portion of this web page)
